Located at the Baillie Henderson Hospital campus within the Health and Wellness Centre
SQRH have established a student-led Health and Wellness Clinic (HaWC) in Toowoomba, located at the Baillie Henderson Hospital within the Health and Wellness Centre.
The clinic provides an interprofessional health and wellness program delivered by nursing and allied health students supervised by experienced health professionals.
Chronic disease issues are prevalent in Queensland and in particular across Southern Queensland. The Health and Wellness Clinic supports individuals with low to rising risk of chronic disease with a program to build the knowledge and skills for participants to live their healthiest life.
What does the program involve?

Please visit your General Practitioner (GP) for a referral to our clinic. Your GP will send your referral to us via Medical Objects or email.

To refer your patient, please complete the below form and include with your referral.

To join the program, a referral from a GP is required. Please discuss with your GP if you are interested in joining us.
